Statewide Medicines Formulary (SMF)

The WA Statewide Medicines Formulary (SMF) is a single list of approved medicines which can be initiated by prescribers across the WA health system, along with any restrictions and guidance that may be in place for the prescribing of that medicine. Its composition is evidence-based and considers the clinical efficacy, safety and cost-effectiveness of medicines. The Statewide Medicines Formulary Policy (MP 0077/18) mandates the initiation of prescribed medicines according to the adult, paediatric and neonatal formularies that make up the SMF.

Formulary One (external site) (F1) is the web-based platform that hosts the SMF. F1 also provides links to relevant resources and alerts where available and shows medicine availability on wards and their location via the imprest functionality.

Who can use F1?

F1 is accessible to WA Health employees including medical practitioners working with public patients in private hospitals.

If you work with public patients in a private hospital in Western Australia, or are a public clinician using a hand held device, access to F1 will require a log-in, using your health employee (HE) number.
