Advance Health Directive Guide consultation

The End-of-Life Care Program is seeking input into the development of a guide to accompany the revised WA Advance Health Directive (AHD).

Community consultation about the development of the new AHD Guide will occur from 31 January 2022 to 18 February 2022. Anyone with an interest or involvement in advance care planning is invited to participate.

Consultation workshops – register now

The workshops offer an opportunity for stakeholders to review the Guide, have face-to-face discussions and provide feedback into the development of the new AHD Guide. There are a range of workshops for specific groups, register or learn more via the links below:

Health professionals – including GPs and clinical staff (external site)
Friday 4 February,7.30am - 9.30am
Open workshop for all, including consumers and carers: individuals and peak bodies (external site)
Wednesday 9 February, 9.30am – 11.30am
Regional, rural and remote health professionals (external site)
Thursday10 February, 12.30pm – 2.30pm

Targeted consultations will be held for people from Aboriginal communities.

Online survey

The survey will be open from Monday 31 January until Friday 18 February 2022 and take approximately 20 minutes to complete.
