Youth friendly health assessment

Trapeze is a comprehensive online resource for The Sydney Children’s Hospitals Network, supporting young people to make the leap from paediatric to adult health services. It includes information for GP’s, Paediatric and Adult teams.
Transition to adult healthcare (external site)
Moving from a paediatric to an adult health service is a transition. The Child and Adolescent Health Service website provides information and resources to help patients and their parents with this process.

As part of the Young and Well Cooperative Research Centre, NSW Centre for Advancement of Adolescent Health developed a better practice guide for the safe, and effective use of technologies to promote young people's wellbeing
