Who is the Interim Hospital Home for?
Brightwater at home IHP services is for anyone 65 years or older living in the community who has acute or post-acute care needs that can be treated effectively and safely in the person’s place of residence. Referrals are generally through a public hospital, GP or community nurse.

Brightwater at Home Interim Hospital Program is available in most of the Perth Metropolitan area.

- Transitional Care
Transitional Care provides you with short term care to assist you with rehabilitating after a hospital admission.
- Commonwealth Home Support Program
The CHSP helps older Australians access entry-level support services to live independently and safely within their own home.
- Home Care Packages
Once you have been assessed and approved for a Home Care Package by the Australian Government, we will work with you to create a personalised Home Care Package, to ensure you receive the care and support you need.
Depending on the program, we can offer cleaning, meals, shopping, social outings, personal care, respite, physiotherapy, occupational therapy, speech pathology, dietetics, social work, podiatry and more.
