Service Agreement Mid-year Review Deeds of Amendment 2017-18 — Abridged

The Service Agreement Mid-Year Review Deed of Amendment 2017-18 for each Health Service Provider represents the changes to the previously executed Service Agreement Budget Deed of Amendment 2017-18 as a consequence of the Mid-Year Review of the WA State Budget in December 2017.

Although PathWest Laboratory Medicine WA (PathWest) is not recognised as a separate Health Service Provider for the period 1 July 2017 to 30 June 2018, a separate Service Agreement Mid-year Review Deed of Amendment was produced to be executed by the Board Chair and Chief Executive of the North Metropolitan Health Service.
