Having people at the centre of care, ensuring access to care when it is needed by the most vulnerable in our community and which is coordinated between hospital, primary care, aged care and disability sectors, and implementing a more coordinated system that will support people to stay well in their community and potentially reduce demands on emergency departments and unnecessary costs are key to improving sustainable health care in Western Australia.

Improve timely access to outpatient services

Priorities for 2024

  • Increase the scope of the Clinical Referral Service to include regional emphasis
  • Develop Referral Access Criteria for medically led outpatient specialities

Achievements

  • Vendors awarded for the Smart Referrals WA project
  • 24 month roadmap for delivery presented to HEC
  • Refreshed the Outpatient Services Policy to reflect projects and prioritise virtual care
Implement models of care
Develop and implement a systemwide approach to identifying and supporting people who are frequent users of health services including emergency and outpatient services to improve pathways of care and reduce presentations.

Priorities for 2024

  • Finalise Community Health Services Strategy
  • Implement phase 1 of the Strategy focussing on older adults and increasing access to integrated services in the community
  • Strengthen partnerships between hospitals and GPs for older people and those with chronic or complex conditions

  • Expanded hospital substitution services in the home, including Hospital in the Home
  • Improved access to high-quality care, closer to home
  • Consultation and collaboration to unpack the community health services landscape, strengths and opportunities
